With using the FADE model the QI team utilizes a four-step process which involves using a repetitive approach, it repeats the steps repeatedly to evaluate how well the plans that have been put into action are going and what are the impacts that are taking place in the changes of the system (Giardino, 2016).

There are four steps of the FADE model which are called the focus, analyze, develop, and execute phases (Spath, 2013). The focus phase is where you choose a problem and write out a statement explaining it (Spath, 2013). Analyze is the phase where you learn more about the problem but obtaining data about the performance that is being questioned (Spath, 2013). Develop phase is coming up with a solution for the problem and planning for implementing the solution that you come up with (Spath, 2013). Finally, the phase to execute which is putting the plan into action and monitoring the results and adjusting the plan if needed (Spath, 2013). The identified practice problem is the inability of ward nurses to finish drug rounds on time.
